AHCCCS
PATH is an enrolled AHCCCS provider, and AHCCCS visits cost you nothing. We accept all six AHCCCS Complete Care plans in Maricopa County: Arizona Complete Health–Complete Care Plan, Banner–University Family Care, Blue Cross Blue Shield of Arizona Health Choice, Mercy Care, Molina Healthcare and UnitedHealthcare Community Plan. We can also see you if you are on AHCCCS fee-for-service without a health plan, or with the American Indian Health Program, Tribal ALTCS or a Tribal RBHA. If you are with a different AHCCCS program, call and we will tell you right away whether we can see you. Bring your AHCCCS ID either way. See every AHCCCS plan we accept
Commercial insurance
We are not in network with commercial insurance plans. You can pay yourself at the rates above, and we provide a superbill on request, which you may submit to your insurer for possible out-of-network reimbursement. Whether you are reimbursed, and how much, depends on your own benefits and is not guaranteed.

Individual therapy

Per session.Sliding scale
We offer sliding-scale rates for patients experiencing financial hardship. Eligibility and the reduced rate are worked out case by case, based on your circumstances. Call and ask; you do not need to explain yourself in writing first.
Good Faith Estimate
If you are paying yourself or using out-of-network benefits, you will receive a written Good Faith Estimate of what your care will cost before your first appointment. That is your right under federal law.
Cancellations
We ask for at least 24 hours' notice to cancel or reschedule. If you are paying yourself, cancelling with less than 24 hours' notice, or not showing up at all, is a $100 fee for an individual appointment or $40 for a group session. AHCCCS members are never charged for a missed appointment. You also get a 15-minute grace period: if you have not arrived or joined by then, your clinician may mark it a no-show. Emergencies and genuinely unusual circumstances are handled case by case.
