We accept every plan listed here for care in Arizona.

- Carelon Behavioral HealthCarelon manages mental health benefits for some health plans. If your card mentions Carelon, we accept your plan through them.Accepted for individual therapy, couples therapy, psychiatric evaluations and medication management. Not IOP or PHP.
- CurativeAccepted for individual therapy, couples therapy, psychiatric evaluations and medication management. Not IOP or PHP.
- TRICARE West, through TriWestAccepted for IOP and PHP only (prior authorization needed, and we handle it). For active-duty service members, National Guard and Reserve members, retirees, and their families. Active-duty members need a referral from their primary care manager; we check on the call.
IOP and PHP are covered through TRICARE West only. For other plans, IOP and PHP are available self-pay or out of network with a superbill.

How insurance billing works
Insurance billing for Aetna, Cigna, Carelon Behavioral Health and Curative is handled by our billing partner, Headway. After you book, you will get an email from Headway to confirm your coverage and add a card for your copay or coinsurance. You will see your estimated cost before your first session.
We ask for your email address when we call you back, so Headway can reach you.
TRICARE West claims for IOP and PHP are billed by PATH directly.

- Sliding scale
- We offer sliding-scale rates for patients experiencing financial hardship. Eligibility and the reduced rate are worked out case by case, based on your circumstances. Call and ask; you do not need to explain yourself in writing first.
- If you need to cancel
- Give us 24 hours. For an individual or couples appointment, a late cancellation is $50 and a no-show is $100. For group, the fee is $40. Our financial and cancellation policy

Four questions we get about cost
Do I need a referral from my doctor?
No, with one exception: active-duty TRICARE members need a referral from their primary care manager, the military doctor in charge of their care. Everyone else can book directly.
How much will my plan pay me back?
We cannot promise what your plan will pay. We check your out-of-network benefits with you on the phone, before you start. How out-of-network benefits work
Why did I get an email from Headway?
Headway is the billing partner that handles claims for Aetna, Cigna, Carelon Behavioral Health and Curative at PATH. After you book with one of those plans, Headway emails you to confirm your coverage and to add a card for your copay or coinsurance. The email is expected, and your estimated cost is shown there before your first session. If anything in it looks wrong, call us and we will sort it out with you.
What do these insurance words mean?
A copay is a set dollar amount you pay for each visit. Coinsurance is a percentage of the bill that you pay. Out of network means your plan has no agreement with us, so you pay us and ask your plan to pay you back. A superbill is the detailed receipt you send to your plan to do that. Prior authorization means your plan has to say yes before care starts; for IOP and PHP we ask your plan for you. IOP is our Intensive Outpatient Program, three hours a day, three days a week. PHP is our Partial Hospitalization Program: more days and longer days than IOP, and you still sleep at home.
