Telehealth Informed Consent
This page explains what telehealth is, how PATH provides it, and your rights. You agree to it when you book a video visit; you can also withdraw it at any time by telling us.
What telehealth is
PATH Health Services · 605 E Broadway Rd, Tempe, AZ 85282 · (480) 637-0295
Telehealth means receiving care from a PATH clinician by secure live video instead of in person. Your clinician can evaluate you, make a diagnosis, create a treatment plan, prescribe medication, and follow your progress the same way they would in our office.
What you can expect
- Visits happen over SimplePractice Telehealth, a HIPAA-compliant video platform. You need a device with a camera, microphone, and internet.
- Your clinician is licensed in Arizona. You must be physically in Arizona during every visit, and we will confirm your location at the start of each one.
- We verify your identity at your first visit with a government-issued photo ID.
- Your records are kept in our electronic health record under the same privacy protections as in-person care. See our Notice of Privacy Practices.
- Prescriptions are sent electronically to the pharmacy you choose.
Possible limits and risks
- Some conditions need an in-person exam, lab work, or a higher level of care. If so, your clinician will tell you and help arrange it. Our Tempe office is available for in-person visits.
- Video can drop or freeze. If a visit is interrupted, we will call you at the number on file and reschedule if needed at no charge.
- No electronic system is perfectly secure. We use encrypted, HIPAA-compliant tools and never record visits.
- Certain medications have rules that may require an in-person visit under federal or state law. Your clinician will explain this if it applies to you.
Your rights
- You may stop a telehealth visit at any time and request in-person care instead.
- You may withdraw this consent at any time without affecting your right to future care.
- You may ask questions about telehealth before or during any visit.
- Your clinician will tell you their name, credential, and license state at the start of care.
Fees
Telehealth visits are billed at the same rates as in-person visits. Self-pay rates are posted on our insurance and prices page; a Good Faith Estimate is available on request. If you use insurance, your plan’s telehealth coverage applies.
Consent
By booking and attending a telehealth visit with PATH Health Services, you confirm that you have read this notice, you understand it, your questions have been answered, and you agree to receive care by telehealth. You also confirm you are at least 18 years old and physically located in Arizona at the time of your visit.
This consent is provided under Arizona Revised Statutes § 36-3602 and is kept in your medical record.
