Privacy Policy
What happens to information you give us through this website. It is separate from our Notice of Privacy Practices, which explains how we handle your health records once you are a patient.
The short version
Effective 15 September 2026.
This website is deliberately built to collect as little as possible. It never asks a health question, and there is no patient form on it at all. When you request an appointment, the button hands you to SimplePractice, the system that holds your records once you are a patient. What you type there goes to them, not through this website. The only form on this site belongs to the careers page, and that one is for job applicants.
What we collect
- Nothing from you as a patient. There is no patient form on this website and no health question anywhere on it.
- We do not currently run analytics of any kind. If we add them they will be limited to marketing pages, will record page views and referral source only, and will never run on the appointment request widget or the client portal.
- No session recording, heat-mapping or screen-capture tool runs anywhere on this site.
When you request an appointment, that request goes to SimplePractice, who hold it under a business associate agreement with us. It is covered by our Notice of Privacy Practices rather than by this policy, and it does not pass through this website on the way.
If you apply for a job
Our careers page has an application form. It collects your name, email address, phone number, the role you are applying for, your Arizona license type and status, your résumé, and anything else you choose to tell us.
That information goes to our Executive Director and is used to consider your application, nothing else. We do not sell it, we do not add you to a marketing list, and it is kept apart from any patient record system. If we do not hire you we keep your application for twelve months in case something else opens up, and you can ask us to delete it sooner by calling (480) 525-3770.
Text messages
This website does not collect your phone number, so nothing here signs you up for anything. If you give us your number when you request an appointment or when you call, we may text you about that appointment. Every message we send ends with instructions to reply STOP. If you reply STOP we stop texting you, immediately and permanently, and it does not affect your care or your ability to book by phone.
We do not send marketing text messages.
Cookies
This site sets no cookies of its own. If we add analytics to our marketing pages in future, those may set one. You can block or delete cookies in your browser settings and the site will still work; nothing on it requires a cookie to function.
Children
This website is intended for adults. We do not knowingly collect information from anyone under eighteen through this site.
Security
The site is served over an encrypted connection. Form submissions travel encrypted. No method of transmission over the internet is completely secure, which is the reason this site asks no health questions.
Links to other sites
Our scheduling and client portal are operated by a third party under a business associate agreement with us. When you use them you are on their system and their privacy terms apply alongside ours. We also link to crisis lines operated by other organisations; those are not ours and we have no access to those conversations.
Your choices
- You can call us instead of booking online. Nothing on this site has to be typed.
- You can ask us what we hold about you, and ask us to delete it, by calling (480) 525-3770. If you never became a patient, we delete it. If you did, there is a limit, explained below.
- You can reply STOP to any text message.
- You can block cookies in your browser.
What we cannot delete
If you contacted us and never became a patient, we delete what you gave us when you ask. If you are or were a patient, your clinical record is different. A.R.S. § 12-2297 requires a health care provider to keep a record for at least six years after the last date the patient received services, and for a patient who was a minor, until three years after the eighteenth birthday or six years after the last service, whichever comes later. Deleting a record inside that window is not something we are permitted to do. You can still ask us to correct it, and you can still restrict who sees it.
If you are visiting from outside Arizona
Arizona has not enacted a general consumer privacy statute of the kind California and a number of other states have, and this practice serves Arizona. Visitors do arrive from states that have one, so rather than gate the choices above by residence, we offer them to everyone who uses this site. If you live in a state with its own privacy law and want to exercise a right under it, call (480) 525-3770 and we will handle the request under that state’s law.
Two of those laws are worth naming. Washington’s My Health My Data Act and Nevada’s Senate Bill 370 regulate consumer health data collected outside the HIPAA system, and both reach companies that serve residents of those states. This site is built so that it collects no consumer health data from anyone. It asks no health question and has no patient form on it.
If there is ever a breach
Arizona’s breach notification statute, A.R.S. § 18-552, exempts entities that comply with HIPAA. A breach involving your health information is therefore handled under the federal breach notification rule, which requires us to notify you without unreasonable delay and no later than sixty days after we discover it. For information from this website that is not health information, the Arizona statute applies and requires notice within forty-five days of our determining that a breach occurred. If a single breach reached more than one thousand people, that statute also requires us to notify the Attorney General, the Director of the Arizona Department of Homeland Security and the three largest nationwide consumer reporting agencies.
Changes to this policy
If we change this policy we will update the effective date at the top. Material changes will be posted on this page.
