Using your out-of-network benefits
Your plan may pay you back for part of what you paid us, depending on your out-of-network benefits. It takes four steps, and we’ll help with every one.
- You pay
- Our self-pay rate for your service, listed under Our self-pay prices further down this page.
- You get
- A superbill after each session, with the codes your plan needs.
- Your plan
- May pay you back for part of it, depending on your benefits.

What out of network means
Out of network means PATH doesn’t have a contract with your insurance company. You can still see us, and if your plan has out-of-network benefits, it may pay you back for part of what you paid.
How it works at PATH
You pay our rate
You pay PATH our self-pay rate. Every price is on this page, so you know the number first.
We give you a superbill
It's an itemized receipt with the codes insurers need. You get one after each session.
You send it to your plan
Your plan tells you how: online, by app or by mail. It's question 5 on the list below.
Your plan pays you back what your benefits allow
The money comes from your plan, not from us. How much depends on your out-of-network benefits and deductible.
What a superbill is
It’s an itemized receipt with the codes insurers need. You don’t have to understand the codes. Your plan does.
- Clinician and NPI
- Date of service
- Diagnosis code
- Service code
- Individual therapy, 60 min
- Amount paid
- $150.00
Who you saw. Your clinician's name, license and provider number.
When. The date of each session.
Why. A diagnosis code, filled in by your clinician.
What was done. A service code for the kind of visit and how long it was.
What you paid. The amount, marked paid.
What to ask your insurance company
Call the member services number on your insurance card and ask these five questions. Write down the answers, the name of the person you spoke with, and the date.
- Do I have out-of-network mental health benefits?
- What is my out-of-network deductible, and how much of it have I met?
- What percentage do you reimburse for out-of-network outpatient mental health?
- Do I need a prior authorization for an intensive outpatient program?
- How do I submit a superbill, and how long does reimbursement take?
We’ll call them with you
We will call your plan with you to check your benefits before you start. You don’t have to figure out the insurance company on your own.
