Nondiscrimination and language help
Everyone gets the same care here. If you need an interpreter, information in another format, or a change to how we work because of a disability, we provide it free of charge.
Our commitment
PATH Health Services complies with applicable federal civil rights laws. We do not discriminate on the basis of race, color, national origin (including limited English proficiency and primary language), sex, age or disability. We do not exclude people or treat them differently because of any of these.
Help for people with disabilities
We make reasonable changes to how we work for people with disabilities, and we provide aids and services so you can communicate with us effectively. These include qualified sign language interpreters and written information in other formats, such as large print or braille. They are free of charge, and we provide them in a timely way.
Language help
If English is not your first language, we provide language help free of charge. That includes qualified interpreters and written information translated into other languages.
How to ask for help
Call (480) 525-3770, or tell us at your appointment. For an ASL interpreter, about a week’s notice gives us time to book one. If your appointment is sooner, call anyway and we will tell you what we can arrange.
If you think we have discriminated against you
If you believe we have not provided these services, or have discriminated against you in another way, you can tell us by calling (480) 525-3770 or writing to PATH Health Services, 605 E Broadway Rd, Tempe, AZ 85282.
